EU5 Disease and Population Loss Explained

Diseases in EU5 spawn on a monthly roll, spread through contact and trade, and can wipe out double-digit percentages of a location's population depending on which disease it is. Here's the mechanic, updated for the 1.3.10 migration fix.

Disease in EU5 isn't a scripted event chain the way EU4's occasional plague incidents were. It's a running background system that rolls monthly, spreads through concrete channels, and can genuinely reshape a region's population if left unmanaged. It was also one of the most-contested balance areas through the 1.2.x and early 1.3.x betas, with players reporting epidemics running out of control before recent patches brought spread duration under control.

How outbreaks start

Diseases spawn semi-randomly around the world, with most types rolling a 10-25% monthly spawn probability wherever their trigger conditions are met. Some diseases, Bubonic Plague specifically, have their own distinct triggering conditions rather than the generic roll other types use. Once a disease spawns in a location, it begins spreading outward from there.

How diseases spread

Spread happens through several concrete channels, not a single "infection rate" number:

  • Person-to-person contact within the same infected location
  • Trade routes: spread scales with trade capacity, with the effect roughly equivalent to what 5 trade capacity produces
  • Adjacency: once a location's infection level crosses a threshold, neighboring locations become exposed
  • Capital cities of already-infected territories, which act as amplified spread points

Each already-infected person attempts to infect somewhere between 1 and 20 previously-uninfected people per spread interval, with the exact figure depending on settlement type and the specific disease. Locations also roll separately to determine whether the outbreak goes locally stagnant, and that stagnation chance rises with distance from the original outbreak point, meaning diseases naturally lose steam the further they travel from where they started, all else equal.

How severe an outbreak gets

Mortality isn't uniform across disease types. This is the single biggest factor in how seriously a given outbreak should be taken:

An Influenza outbreak is close to a non-event at the population level. A Great Pestilence outbreak is a location-defining catastrophe. Check which disease you're dealing with before deciding how much effort to spend containing it. Treating every notification with equal concern is a mistake.

After the initial infection wave, roughly 25% of previously-infected people (for most disease types) roll again to either die or gain resistance, based on that disease's mortality rate, meaning a location doesn't take its full mortality hit all at once; it's an ongoing process across the outbreak's duration, not a single instant casualty roll.

Mitigation

  • Disease resistance: a location or population-level stat that reduces both spread effectiveness and the odds of an outbreak stagnating locally rather than continuing to spread.
  • Restricting outside contact ("forbidding contact with outsiders" or equivalent trade/travel restriction actions) raises stagnation chance by roughly 50 percentage points, a substantial containment lever, at the obvious cost of the trade and diplomatic activity you're restricting.
  • Avoiding unnecessary market/trade exposure during an active outbreak: since spread scales with trade capacity and market presence, a location actively trading heavily during an epidemic is working against its own stagnation odds. The threshold where it starts really hurting

Beyond the direct mortality figures, infection carries its own escalating penalties once it crosses roughly 20% of a location's population: promotion speed drops sharply (on the order of −200%) and monthly prosperity takes a substantial ongoing hit (around −10%). This means the practical cost of a bad outbreak isn't just the eventual death toll. It's months of degraded location performance while the outbreak is active and infection stays above that threshold, even before counting the population actually lost.

What changed in the 1.3.x cycle

The 1.3.10 beta patch (immediately preceding the current 1.3.11 stable) specifically addressed one of the most-reported disease complaints from earlier in the year: locations with pops actively migrating in no longer keep a disease alive indefinitely. Earlier versions had a bug-adjacent interaction where ongoing migration into an infected location could keep re-seeding the outbreak past the point it should have burned out or gone stagnant, a problem forum threads from the 1.2.2/1.2.3 era specifically flagged as "out of control epidemics."
